- As a new patient, please obtain a signed prescription from your referring physician. Bring the prescription with you to your first appointment, or you may have your doctor fax it to us prior to your appointment.
- In some cases, your insurance may require a referral in addition to the prescription. If you are unsure, call our office at 804.379.3002 and we will help you determine what you need.

- Please arrive a few minutes early to fill out forms, or print the necessary forms below, fill them out, and bring them to your first appointment.
*For an orthopedic diagnosis of back pain, osteoporosis, knee/foot/ankle dysfunction, fibromyalgia, hip pain, thoracic pain, etc., please complete Personal Information, Subjective Evaluation, HIPPAA Form, and Billing Procedures.
*For a urogynecology diagnosis such as pelvic floor dysfunction or chronic pelvic pain, incontinence, constipation, anal pain, painful menstrual periods or intercourse, inflammation of the bladder, pain of the vulva, etc., please complete Personal Information, Subjective Evaluation, HIPPAA Form, and Billing Procedures.
